Creating your Services contract 

Once you have understood what you are looking for and which supplier best meets your needs, the next step should be to agree on a schedule with your supplier. A clear and concise employment contract will help both parties have a smooth cooperation and future work journey. An effective employment contract should include the following essential contents. 


(a) Description of support and services to be provided 

  • Explicit mention of your goals that the provider will help you achieve.
  • The type of support provided
  • Time and place when or where services will be provided
  • Cost of the supports
  • Duration for which these supports will be rendered


(b) Statement of duties and responsibilities clearly 

  • The service agreement must have a clear statement by the provider about their responsibilities under the service agreements that are in accordance with your expectations and support goals.
  • It should also include the ways or methods that can be used to change or end your agreement.
