  • As a participant, you may belong to any cultural background; make sure your provider provides multilingual support staff that helps you understand yourself well and fulfil your needs.

It may be possible that you are already working with a provider, and it usually seems easy to stick to the one you are already working with. Devising your Service Agreement

Once you have clearly understood what you are looking for and which provider best suits your needs, the next step should revolve around the planning agreement with your provider. A clear and concise service agreement will help both parties to have a smooth journey to mutual cooperation and work ahead. An effective service agreement should include the following basic contents. 

        

(a)   Details of Supports and Services to be Delivered 

  • Clear mention of your goals that the provider will help you achieve.
  • Type of the support provided
  • Time and place when or where services will be provided
  • Cost of the supports
  • Duration for which these supports will be rendered

(b) Clear Mention of roles and responsibilities 

  • Service agreements must have a clear statement by the provider about their responsibilities under the service agreements that are in accordance with your expectations and support goals.
  • It should also include the ways or methods that can be used to change or end your agreement.
